2016 Louisiana Caring Communities Youth Survey

Please use the links below to view the anonymous questionnaires to be used in Utah's Student Health and Risk Prevention (SHARP) Survey administration. SHARP is a completely anonymous survey.

A majority of participants will be taking the Prevention Needs Assessment (PNA) form (either Form A or Form B). Most participants will take this survey online, though a paper format will still be available.

A small number of high school students (grades 9-12 only) will be taking the Youth Risk Behavior Survey instead of the PNA.

Students who have parental consent will only take one anonymous survey form in the SHARP administration. Students who do not have parental consent will not take the survey in any format.
